Downloadable NUANS® Subscriber Kit

Follow these simple steps to download the NUANS® Subscriber Kit:

1. You will need to download, print and complete all forms specified below.

2. Review all completed forms.

3. Ensure the following three completed forms are signed and returned to the NUANS Administrator:
   
   • Subscriber Kit Application Form
     
   • Subscriber Credit Card Authorization Form
     
   • NUANS Services Contract (two copies are required)
