Downloadable NUANS® Subscriber Kit

Follow these simple steps to download the NUANS® Subscriber Kit:

1. You will need to download, print and complete all forms specified below.

2. Review all completed forms.

3. Ensure the following four completed forms are signed and mailed to the NUANS Administrator:
   
   • Subscriber Kit Application Form
     
   • Subscriber Credit Card Authorization Form
     
   • NUANS Services Contract, a copy for NUANS records
     
   • NUANS Services Contract, a copy to be signed by the NUANS Administrator for your records
